2016 PHI MultiPak Training Attendee Information

Payment/Billing Information

The fee for the MultiPak Training Session for XPS is $350 (this training is limited to the first 15 paid registrations). There are two ways to pay: credit card (download attached form) or purchase order / invoice.

Invoices will be sent approx. 4 weeks prior to class, and must be paid no later than May 2, 2016.

IMPORTANT:

The fee includes the training, all coffee breaks, and lunch. Please note that all participants are required to bring their own laptop computer to the class. Participants are responsible for all other expenses including their own transportation and hotel costs, if applicable.
